    Order Forms English Literature Order Forms (PayPal) Please submit an "Application to Handle Literature" if we do not have one on file. All lessons and certificates are stocked in packages of 25, except the 8-page flat lessons which are in packages of 50. Reimbursement is expected for the cost of materials and postage unless other arrangements have been made. All payments should be made in U.S. Dollars.
Actual shipping charges will be invoiced. This order form will calculate estimated shipping charges at check-out based on the following formula.
International orders should add 30% to the order total. Please use the "Donate additional amount" to the SLM Int. Mininstry to send the balance.
